Fixed term contract until 31 March 2026
An exciting new opportunity has arisen for 2 x Horizons Prevention Workers within the Horizons Team. This is fixed term position until 31.03.26, working with children and young people who are at risk of entering the youth justice service. The service has great working relationships with partners and offers a truly multi-agency way to improving the outcomes for children and young people
The post holders will be responsible for: -
- Chairing and co-ordinating Team around the Family (TAF) meetings with families and partner agencies to devise an outcome focused plan to bring about change for the family.
- To undertake direct work with children, young people and families with a specific focus on behaviours for young people that are increasing their risk of coming into the youth justice arena.
- To work with families and partner agencies to devise, implement and review an outcome focused plan and to bring about better outcomes for Children and Families.
- Keep accurate and up to date recordings on EHM of all contact, meetings and interventions with children, young people and families.
You will need to have: -
- Two years’ direct experience of working within a child, young people or families setting where a caseload has been held.
- NVQ Level 3 or above in children, young people and families field.
- Experience of working as part of a multi-agency team, preferably in Youth Justice or family support setting.
- Experience of undertaking assessments, devising and reviewing plans and delivering creative direct work/ interventions.
